At Foot and Ankle Specialists, we feel honored to serve our patients. Our goal is to work in partnership with the community of patients who’ve entrusted us with their medical care. In order to meet your expectations as our patient, please review the following information to enhance your experience in our office. Our frequently asked questions page provides answers to most questions regarding office appointments. New Patient Forms

Online Patient Portal

Our office uses an electronic medical record system. This allows our patients to access their medical records and make changes directly to the system. When you make your first appointment, just provide an e-mail address and we will send you a link with login directions to access your medical record and add the necessary information before your appointment.

Find Us

Visit our Contact page and select the map. A link will open in Google Maps. Driving directions can be found by typing in your current or starting location and step-by-step instructions will bring you right to us. Or, just give us a call: (801) 569-2696.

What to Bring

  • Current copy of your medical insurance card
  • Form of payment for any copay
  • Photo ID
  • List of prescription medications
  • List of vitamins and supplements you currently take.
  • Prior medical records including laboratory results, x-rays, or copies of MRI’s or CT scans.

What to Wear

Most foot and ankle examinations don’t require special attire. However, in some instances, a more complete biomechanical examination is required and tight-fitting clothes or shorts work best. Some patients find it useful to bring their current orthotics or specific shoes related to their foot or ankle problem.

Insurance

We accept most insurance plans. There are a few exceptions, so we encourage you to ask the scheduling desk about your particular insurance plan when you make an appointment.

Existing Patients

Online Patient Portal

Our office uses an electronic medical record system. This allows our patients to access their medical records and make changes directly to the system. When you made your first appointment, and if you provided us with an e-mail address, then a patient portal has been created. If you did not receive a link for the portal access, please contact our office and we will assist you in accessing your medical record. Through this portal, you can update medications, add pictures, and even communicate directly with us via a secure link.
